Retropharyngeal abscess and stridor in infants.

چکیده

Abscessos retrofaríngeos são geralmente resultantes de processos infecciosos envolvendo as vias aerodigestivas superiores, bem como processos traumáticos, como ingestão acidental de corpo estranho ou intubação orotraqueal traumática. Acometem, na maioria dos casos, crianças de até cinco anos, acarretando sintomas iniciais inespecíficos como irritabilidade, inapetência e febre; com a progressão do quadro observa-se disfagia, odinofagia, sialorréia, voz de “batata quente” ou choro abafado. A sintomatologia está diretamente relacionada ao volume do abscesso, podendo haver grave comprometimento respiratório. O quadro clínico pode ser dramático, uma vez que a formação do abscesso no espaço retrofaríngeo faz com que haja aproximação do mesmo das estruturas do palato mole, base de língua, parede posterior da faringe e laringe estreitando, consideravelmente, o lúmen da via aérea, podendo cursar com insuficiência respiratória, em especial, em lactentes. Uma vez confirmado o diagnóstico de abscesso retrofaríngeo, a criança deverá ser imediatamente hospitalizada para monitorização permanente dos sinais vitais e início da terapia medicamentosa que deverá incluir antibioticoterapia endovenosa e drenagem cirúrgica. A ruptura espontânea do abscesso pode levar à aspiração maciça de secreção purulenta e, conseqüentemente, ao desenvolvimento de empiema pulmonar, além de outras complicações como mediastinite e septicemia. A permeabilidade das vias aéreas deverá ser assegurada por meio de intubação orotraqueal ou traqueotomia.
